Health Agency Los Angeles

The Contest Committee, in its sole discretion, shall have the ability to distribute any prizes among multiple contestants if the Committee determines that a particular logo design or motto is the same or very similar to other entries that have been submitted for this contest.

Logo Contest Guidelines

Summary:

The Health Agency executive leadership team is inviting submissions for a logo design to support the branding of our Health Agency. The winning design will be used in printed and electronic formats. The contest aims to drive internal enthusiasm and support for our Health Agency that will transform the delivery of health care amongst all of our Health Agency Departments.

Guidelines:

The contest is open to all part-time and full-time permanent DHS, DMH, and DPH employees, and volunteers. Entries can be drawn freehand or created in software programs that produce the .JPEG file format. Designs that support the Health Agency values are encouraged.

  • Designs drawn by hand can be produced on any grade of paper using any combination of colors and color instruments, such as crayons or markers.
  • Designs that are submitted electronically in a format other than .JPEG cannot be viewed and will not be considered. Designs created in programs requiring special software, purchases or licenses to view entries will also not be considered.
  • ALL submissions become the property of the Los Angeles County Health Agency which maintains the right to professionally alter any submission (such as color, size, or concept) for business or commercial use.

How to Enter

Timing

The contest runs between October 15, 2016 and December 30, 2016. All entries must be received by 5 PM on December 30, 2016. All submissions must include 1) name of employee; 2) Member ID number

Review of Entries:

A contest committee comprised of representatives from SEIU 721, Teamsters, and Health Agency Executive Management will review all entries and select ten (10) Semi-Finalists to be announced in January. The 10 semi-finalist entries will be posted on the contest website for voting by all Agency employees. The tallied top-five (5) Finalists will be invited to a recognition luncheon hosted by Health Agency Director Dr. Mitchell Katz, Health Agency Deputy Director, Dr. Alexander Li, and members of the Health Agency executive team. One (1) Grand Prize Winner whose design receives the highest number of staff votes will be announced at the luncheon.

 

Grand Prize:

The Grand Prize winner will receive an iPad. All Finalists will also be presented with certificates and will appear in the DHS staff newsletter .

Motto Contest Guidelines

 Summary:

The Health Agency executive leadership team is inviting submissions for mottos to support the branding of our Health Agency. The winning motto will be used in printed and electronic formats. The contest aims to drive internal enthusiasm and support for our Health Agency that will transform the delivery of health care amongst all of our Health Agency Departments.

Guidelines:

The contest is open to all part-time and full-time permanent DHS, DMH, and DPH employees, and volunteers. Mottos that support the Health Agency values are encouraged. While there is no specific word limit on the motto, entries should be succinct to the extent possible. Some examples of branding mottos include “Moving Care Forward” (ORCHID motto), “The Ultimate Driving Machine” (BMW), “Healthier Tomorrows Start Here” (Kaiser Permanente).

  • ALL submissions become the property of the Los Angeles County Health Agency which maintains the right to professionally alter any submission (grammar, spelling, etc.) for business or commercial use.

In the event that duplicate mottos are received and the motto is chosen as one of the 10 semi-finalist entries, a random drawing amongst the motto contestants will be held to select the individual employee who will represent that motto as one of the 10 official semi-finalists in this contest.
